All applicants for licensure in North Dakota are required to provide verification of their examinations and/or licenses held in other states or jurisdictions.
Verification(s) must be submitted for each applicable exam and license as part of the application process. It is the responsibility of the applicant to ensure that all required verifications are requested and submitted.
Applicants may request that their verification be transmitted through NCEES by either using their NCEES record or NCEES’s online verification service. Applicants are responsible for ensuring that their verification requests are sent to the appropriate state board and that their NCEES Record is complete and transmitted to North Dakota.
If an applicant is not using NCEES’s services they must use our approved form. North Dakota will not accept verifications completed on any other forms.
Applicants must complete the top portion of the verification form, including the name and address of the board completing the verification, as well as their own name, address, city, state, ZIP code, and the last four digits of their Social Security number.
After completing this section, the applicant must forward the form to the board(s) where their examination(s) were taken and/or where their licensure was issued. The originating board will complete the remainder of the form, sign it, affix its seal, and return it directly to this office.
The verifying board may submit the completed form by one of the following methods:

Verification forms must be submitted directly by the board completing the verification. Forms submitted by the applicant will not be accepted.
