No, you can create your own log as long as it contains the information required by NDAC 28-04-01-04.

Yes, disciplinary action still may take place. It is your responsibility to keep your information updated with us. We send out multiple emails and/or letters to the addresses we have on file.

If you fail to complete the audit, a case will be opened with the L&I Committee, and they will decide the direction to take.
